Interoception: feeling the instrument you live in
Interoception is the sensing of signals from inside the body. ADHD is associated, in some studies, with differences here. Whether stimulant treatment improves that sensing remains an open question.
Interoception is the brain and nervous system’s perception and interpretation of signals arising from inside the body. Hunger. Thirst. Fatigue. Tension. Heartbeat. Breath. Temperature. Pain. The need to move, to be quiet, to sleep. Emotional arousal as it is felt, not as it is named.

ADHD is associated, in some studies, with differences in interoception — the sensing of internal bodily signals. Findings are mixed and samples are often small.

Kutscheidt and colleagues (2019) found that a small group of adults with ADHD performed worse than comparison adults on a heartbeat-tracking task. The sample was tiny. Heartbeat tracking is a contested measure. Later work has been mixed. That is what “emerging” means: enough to ask a better question, not enough to print a factoid.

One plausible possibility is that improved attentional discrimination gives some people more capacity to recognise internal signals that were already present. Current research supports interoceptive differences in ADHD and supports changes in attentional regulation with treatment. The complete pathway from stimulant treatment to improved interoceptive awareness has not been established.

Some people describe that after ADHD treatment their body became louder — hunger arriving as an event, heartbeats noticeable, clothes intolerable. One plausible reading is that the signals were already there and attention can now reach them. Another is that the medicine, the sleep loss, anxiety, or withdrawal is producing new sensation. A third is that several of these are true at once. Increased awareness is not automatically improvement, and it is not automatically harm.
Technical detail
Interoceptive research distinguishes accuracy (can you detect a heartbeat?), sensibility (do you notice bodily signals in daily life?), and insight (how well does confidence match accuracy?). These are not the same measurement. This site records sensibility as self-documentation. It does not compute an interoceptive accuracy score.
